Trials were not powered for mortality; this is both the safety and the only mortality-efficacy comparison available. |
Gaps
- No efficacy data for any typhoid vaccine in adults over 55, pregnant women, immunocompromised people or travellers: the 2018 Cochrane review states none exist and nothing newer was found. Traveller recommendations rest on endemic-country trials.
- No controlled Ty21a or Vi polysaccharide efficacy estimate in children under 2 years (Vi-PS is not immunogenic below 2 and Ty21a is not licensed below 6); only TCV has under-2 data (Malawi 9 months to 2 years, Bangladesh under 2).
- Ty21a dose-comparison trial (two vs three vs four doses, PMID 2647863) has no abstract on PubMed, so the per-dose figures could not be compiled; the one-dose and two-dose Chilean figures from PMID 2180234 are compiled instead.
- No typhoid burden record is specific to high-income countries beyond US case counts and the share acquired abroad; a case-fatality estimate for travellers treated in high-income hospitals was not found.
- No vaccine efficacy record against severe outcomes (hospitalisation, perforation, death): all trials use blood culture-confirmed typhoid as the endpoint and were not powered for severe disease. The Cochrane 2025 all-cause mortality RR is the only mortality comparison.
- The WHO typhoid position paper (WER 2018) could not be snapshotted from iris.who.int, so no WHO-stated burden or efficacy figures are compiled; the CDC Yellow Book stands in as the expert-body source.
- Under-5 mortality from typhoid specifically (GBD age-stratified deaths) is not in any cached abstract; the children-15-and-under pooled CFR (typhoid-bur-027) is the closest record.
Flags for review
- The WHO typhoid position paper (WER 2018, WHO-WER9313) could not be snapshotted: iris.who.int returned a 6-character DSpace stub for every PDF URL tried and the who.int landing page returned 404. No WHO-stated number is compiled. A human should fetch the PDF manually and add the WHO burden and efficacy statements.
- typhoid-eff-036 (Zimbabwe, 6 months to 45 years, facility controls) has an abstract interval of 26-77 around a point estimate of 46 that looks asymmetric and may be a typographical error in the source (the lower bound is plausibly negative). Recorded as printed; check the full text before publishing.
- The real-world TCV meta-analysis (PMID 41125003) prints the interval for VE against suspected-plus-confirmed typhoid as 0.52-57%, an evident typographical error, so that estimate was not compiled; only its culture-confirmed and XDR estimates are included (typhoid-eff-057, typhoid-eff-058).
- Several burden denominators are hospital- or surgery-based (typhoid-bur-012, -017, -021, -028 to -033, -040) and must not be multiplied with community incidence or presented beside the GBD case fatality (typhoid-bur-004) without labelling. The perforation CFR applies only to the roughly 1.7% of cases that perforate.
- The three Bangladesh TyVOID waning estimates (typhoid-eff-018 to -022) and the Nepal TyVOID estimates (typhoid-eff-011 to -013) are not independent of the primary trials they extend; the page should not count them as separate studies.
- typhoid-eff-014 and typhoid-eff-016 carry 97.5% confidence intervals, not 95%, as the Bangladesh trial used two primary endpoints. The renderer will label them as if 95%; the measure field does not distinguish.
- Conflicting Ty21a pooled estimates: Cochrane 2018 gives 50% cumulative 3-year efficacy for three doses (typhoid-eff-041) while Batool 2024 gives 45% (typhoid-eff-051) and the Chilean liquid-formulation trial gives 78% over five years (typhoid-eff-063). Formulation and dose schedule explain much of this; present them side by side rather than choosing one.
- typhoid-eff-068 (PedaTyph, 100% efficacy with CI 97.6-100) is as printed but comes from a small unadjusted cluster study rated very low certainty by Cochrane; consider displaying the Cochrane re-analysis (typhoid-eff-047) in preference.
- The XDR effectiveness figures (typhoid-eff-028 and typhoid-eff-058) come from the same Hyderabad cohort; the meta-analysis adds no independent evidence.
- typhoid-eff-037 and typhoid-eff-038 are pre-post ecological attack-rate comparisons from Harare that the authors label vaccine effectiveness; secular trends and outbreak cycles are uncontrolled, so they should be shown as weaker evidence than the matched case-control estimates from the same city.
- The CDC Yellow Book snapshot is the 2026 edition page (dated April 2025); its global figures (9.2 million cases, 133,000 deaths) differ from GBD 2017 (14.3 million, 135.9 thousand) because they draw on different models and years. Both are compiled; label the source and year.
