Zoledronic acid (bisphosphonates) and Osteonecrosis of the jaw: is the FAERS signal real?

n=2,708 co-reports · PRR 64.0 · ROR 68.6
A written Bradford Hill assessment of one drug–event pair. The grade is a judgement about the signal, not a statement that this drug causes this event in any one person; FAERS counts are reports, not rates.

Zoledronic acid (bisphosphonates) ↔ Osteonecrosis of the jaw

n=2,708 co-reports · PRR 64.0 · ROR 68.6

Intravenous bisphosphonates such as zoledronic acid can cause osteonecrosis of the jaw, a painful, often persistent exposure of bone that occurs mainly in cancer patients receiving frequent high doses and is associated with dental procedures, poor oral hygiene, and steroid use. The risk is genuinely drug-related and is why dental review before starting therapy and attention to oral health are advised. The 2,708 co-reports (PRR 64.0) reflect the potency of zoledronic acid and its heavy use in oncology.

Regulatory basis: zoledronic acid prescribing information (osteonecrosis of the jaw warning; dental review advice). Established bisphosphonate-ONJ literature.

Bradford Hill viewpointStatusNote
Temporality (necessary)metJaw osteonecrosis appears during therapy, often after a dental event, and may not heal
StrengthmetPRR 64 on a large co-report count
ConsistencymetReproduced across bisphosphonate and denosumab studies
SpecificitymetA recognised antiresorptive-class effect
Biological gradientmetDose- and frequency-related; worse with steroid use and dental procedures
PlausibilitymetImpaired bone turnover in the jaw is the mechanism
CoherencemetCoheres with the labelled dental-review and oral-hygiene advice
ExperimentmetA randomized trial recorded osteonecrosis on zoledronic acid
AnalogymetAnalogous to ONJ with other bisphosphonates and denosumab

✓ Trial evidence found — Zoledronic Acid in Treating Patients With Metastatic Breast Cancer, Metastatic Prostate Cancer, or Multiple Myeloma With Bone Involvement (NCT00869206), N=1,747

Term (as recorded)Arm I (Zoledronic Acid Every 4 Weeks) (n=879)Arm II (Zoledronic Acid Every 12 Weeks) (n=868)
Osteonecrosis — Musculoskeletal and connective tissue disorders44

Osteonecrosis was recorded in 4 of 879 patients on the every-4-weeks zoledronic-acid schedule and 4 of 868 on the every-12-weeks schedule. The trial had no untreated arm, so it shows ONJ occurs on the drug — the basis of the labelled risk — rather than quantifying the excess; notably, the less frequent schedule did not have fewer events.
